Annual health and medical record: This form shall be filled out yearly by all members.
Crew 187 Contact Information: This form shall be filled out yearly by all members.
Crew Fundraising Event Log sheet: This form shall accompany all deposits.
Crew Funds Needed: This form is to be filled out a week in advance if crew members want to use funds for an event or to buy camping equipment.
Summer Packing List: This list is a guide line to be used when camping in the summer. Other equipment may also be needed.
Winter Packing List: This list is a guide line to be used when camping in the winter. Other equipment may also be needed.
Tour Planning Sheet: This form is to be filled out 2 weeks prior to an event outside the Metro area.
Base Camp Waiver Form: This form must be filled out and brought to every event.
Base Camp Facility Permit Application: This should be filled out 2 weeks prior to event.
